Effective October 1, 2026, UnitedHealthcare will no longer require prior authorization for three chiropractic spinal manipulation codes when services are provided to members enrolled in UnitedHealthcare Medicare Advantage or Dual Special Needs Plans (D-SNPs).

98940 — Chiropractic manipulative treatment, one to two spinal regions

98941 — Chiropractic manipulative treatment, three to four spinal regions

98942 — Chiropractic manipulative treatment, five spinal regions
Important Note:
This update applies only to UnitedHealthcare Medicare Advantage and Dual Special Needs Plans.
It does not apply to UnitedHealthcare commercial plans.
At this time, these are the only chiropractic CPT codes included in this prior authorization update. Practices should continue to verify member eligibility, benefits, and authorization requirements before providing services.
